You might be too aware of your surroundings, people, etc.

You need to use a laser-like focus when doing your clinicals. Block out all other distractions.

Also. It is illegal to scribble through documentation to the point that it can't be read. I learned that today. I noticed my instructor staring at me out of the corner of my eye. Then she came over and pointed at my paper and said:

See this? You can't EVER do this on real documentation. You must put a small line through it and write error above it. And another thing, see all the mistakes and scribbles you made? Accuracy. Don't make errors to begin with.
