First of all, carry an aspirin pill in your wallet. When you feel a heart attack coming (chest pains, shortness of breath, etc), chew the aspirin, swallow, and call 911, and say that you want to go to one of the following hospitals in your area (top ranked for cardiology).

As I lay on the floor, the only thing that saved me was a 15-20 year old nitro.
They said it was probably only 35% efficient but that was enough for me to walk away without any permanent damage to my heart.
A family doctor would probably prescribe it if you are overweight and concerned.
You could ask for a one time prescription of 2 pills.

It’s better to get treatment fast than get top notch treatment.
You can always transfer hospitals once stabilized.
Not a bad idea to carry an aspirin. Though, based on metabolic health, I am at fairly low risk of a heart attack.

Medicaid is good about paying for respiratory treatments so you should be good.. ask for Spiriva inhalers and antibiotics like Zpak when it hits you.
